Step 1: Connect with a Registered Nurse
Cherished Transitions’ personalized process starts by connecting with a Registered Nurse (RN) when you submit our short form or calling us directly. You’ll speak directly to an RN who can address your needs and concerns. They will gather information about yourself or your loved one’s current situation, clinical needs and personal preferences. A time will be scheduled to personally meet with you or loved one’s family to perform an RN clinical assessment to evaluate level of care needs and develop an Individualized Care Profile to help identify which senior living communities are the best fit to meet appropriate level of care needs, budget and individual preferences.
Step 2: Community Research
Our RNs thoroughly review the Individualized Care Profile criteria gathered during the clinical assessment. Then the database is researched to determine specifically which Kansas City senior living communities meet your loved one’s overall needs.
Step 3: Community Tours
Together we create a tour plan and itinerary of the top 3-4 communities chosen by you and/or your family. Our RNs contact, coordinate and schedule personalized tours at those chosen communities eliminating those intrusive sales & marketing calls to you directly. We are your trusted one point of contact to coordinate this entire process.
